If you need help completing an application, contact the BSSC office at ced.bsscgrp@ky.gov. A BSSC staff member will work with you to complete a BSSC application at NO COST TO YOU.

To ensure you are downloading the most recent form you may need to delete your temporary internet files before downloading. Our office will not process applications or documentation unless it is submitted on the most recent form. Please check for the current Fiscal year date on the forms.
